Jim Harbaugh practically galloped through the locker room at the 49ers’ practice facility in Santa Clara, Calif. on Friday, a day after undergoing a minor procedure for an irregular heartbeat that kept him away for all of half a day.
Doctors talked to Harbaugh about improving his diet and cutting down on caffeine. He said he also will take aspirin and other medication. Whether he could need a follow-up procedure down the road, Harbaugh doesn’t know.
Harbaugh said QB Alex Smith’s status for Monday is still unknown as he works back from a concussion, and he will be evaluated again by a neurologist. Backup Colin Kaepernick is getting a “little bit more work than normal,” Harbaugh said.
Cutler, Vick ruled out with concussions • Bears quarterback Jay Cutler will miss Monday’s game at San Francisco because of a concussion, meaning Jason Campbell is the likely starter against the 49ers.

Eagles quarterback Michael Vick also will miss Sunday’s game versus the Washington Redskins due to the effects of a concussion. Rookie Nick Foles, who replaced Vick on Sunday and practiced with the first team all week, will get his first NFL start.
Fines • Jacksonville defensive tackle Terrance Knighton was fined $25,000 for making helmet-to-helmet contact with Colts quarterback Andrew Luck.
• Jaguars safety Dawan Landry was fined $10,000 for a fourth-quarter hit on Luck that made contact to the head and neck area after Luck took himself out of the play by sliding feet first.
• Minnesota linebacker Jasper Brinkley was fined $21,00 for a helmet hit on a defenseless Lions player.
• Referee Tony Corrente was fined one game check , about $9,000, for uttering some obscenities with his microphone on during a game Nov. 4 between the Colts and Dolphins.
